Home blood pressure monitoring is a self-monitoring tool that can be incorporated into the care for patients with management of hypertension and control their blood pressure. There are many patients who suffer from gaps amongst some patients that fail to control their blood pressure. By offering remote blood patient monitoring of blood pressure, it will aid in improving a gap in health care barriers for example: transportation to appointments, avoid white coat hypertension, and patient engagement in managing BP. The purpose of this project will be to explore the impact of remote patient BP monitoring and the reduction of BP in patients within the South Florida community. Use the above project of idea to answer the following questions: 1. Briefly describe the phenomenon of interest and the problem you identified that provided the foundation for your project idea. 2.
